Warriors on the Run Color 5K Registration Form 

    Please complete this form to register for the Color 5K.  One form is required for each participant. Once you submit the form, you'll be able to add the registration fee to your shopping cart, have the opportunity to register more runners, and purchase bandanas, sunglasses and extra color packets for an awesome, colorful time on October 2!

    Waiver & Release from Liability

    PLEASE READ CAREFULLY BEFORE SIGNING.
    I know that running a Color 5K Run is a potentially hazardous activity. I should not enter and run unless I am medically able and properly trained. I agree to abide by any decision of a race official relative to my ability to safely complete the run. I assume all risks associated with running a Color 5K Run, including, but not limited to tripping and falling, contact with other participants, the effects of weather, including heat and humidity, defective equipment, traffic and the conditions of the road, and any hazard that may be posed by spectators or volunteers, all such risks being known and appreciated by me. Having read this waiver and knowing these facts and in consideration of acceptance of my entry, I, for myself and anyone entitled to act on my behalf, waive and release WVHS and School District 204, its subsidiaries and affiliates, The WVHS PTA, The WVHS Athletic Booster Club, The State of Illinois, Illinois Dept. of Transportation, The City of Aurora, all sponsors, contractors and volunteers, their representatives and successors from all claims or liabilities of any kind arising out of my participation in this event even though that liability may arise out of negligence or carelessness on the part of the persons named in this waiver. I grant permission to all of the foregoing to use any photographs, motion pictures, recordings, or any other record of this event for any legitimate purpose and I understand that my name will be posted on WVHS PTA, Booster Club or District 204 websites as a participant of the Color 5K Run. I also understand that my entry fee is non-refundable. A parent must sign if entrant is 18 years of age or younger. This is to certify that my child has permission to compete in the Color 5K Run, is in good physical condition and the event officials may authorize necessary medical treatment. I understand that bicycles, skateboards, baby strollers, roller-skates or blades and animals are not allowed in the race and I will abide by this guideline.

    By entering my name below to sign this waiver electronically, I assert that I have reviewed and agree to the above waiver.  If the entrant is 18 years of age or younger, parent must agree and electronically sign.
