PARTICIPATION WAIVER AND RELEASE OF ALL CLAIMS & ASSUMPTION OF RISK
Waubonsie Valley High School and the WVHS Athletic Booster Club

Please read this form carefully and be aware that registering for participation in the Warriors Dodgeball Tournament, you will be waiving the rights to all claims for injuries the participant might sustain arising out of this event and you will be required to indemnify, hold harmless and defend WVHS Athletic Booster Club, Waubonsie Valley High School, and Indian Prairie School District #204, for any claims arising out of participation in the Warriors Dodgeball Tournament, Sunday, February 2, 2025 sponsored by the WVHS Athletic Booster Club.

The participant has read and understands the rules and regulations of the Warriors Dodgeball Tournament 2025 and understand that any violations of the rules provided could/will result in the disqualification of their participation. The entire team will report to the WVHS gym on Sunday, February 2, 2025 (10:30am for check in). All rules for the tournament will be discussed prior to the first match. The entry fee is non-refundable.

I give my child permission to participate in the Warriors Dodgeball Tournament held on Sunday, February 2, 2025. I understand that they have been provided with all the rules and regulations of the dodgeball tournament and I acknowledge that any violations of these rules could/will result in the disqualification of my child. I also understand that I/my child understands all the risks of playing in the Warriors Dodgeball Tournament and will not hold the WVHS Athletic Booster Club, Waubonsie Valley High School, and Indian Prairie School District #204, or any person playing or refereeing the dodgeball tournament responsible for any injuries incurred during the duration of the event. I also grant permission to all of the foregoing to use any photographs or video footage of this event for any legitimate purpose.

A parent must sign if participant is under18 years of age. This is to certify that my child has permission to compete in the Warriors Dodgeball Tournament, is in good physical condition and the event officials may authorize necessary medical treatment. By entering my name below to sign this waiver electronically, I assert that I have reviewed and agree to the above waiver for my child to participate in the Warriors Dodgeball Tournament.
