TEAM REGISTRATION FORM

Please fill out this form to electronically register your team for the WVHS Dodgeball Tournament held on Sunday, February 2, 2025. Only ONE team registration form is needed per team and should be filled out by the team captain. The team captain is also in charge of collecting the player entry fees for their team and will submit an online payment for their entire team by Monday, January 20, 2025. Please remember that ALL participants must complete a player waiver form.
